Refined Vertical Neck Lift With Crisscrossed Platysma Traction Sutures: A Simple Method for Improving Mandibular Neck Definition in Face Lifts.

Abstract

[INTRODUCTION] Jawline and upper neck definition are crucial for aesthetically pleasing outcomes in face lifts. Still, neck rejuvenation is challenging with current techniques which can be either invasive or have minimal and short-term effects. Building upon known methods, we report on a series of patients utilizing a modified suturing technique to perform neck lifting and jaw line contouring based on submental crisscrossed platysma traction sutures. This technique is safe, reproducible and can be combined with any facelift procedure.

[METHODS] This series included 154 patients who underwent face and neck lift with the proposed method. After elevation of a skin neck flap through a retro auricular incision and dissection of medial platysma borders through a midline submental incision, medial platysma borders are crisscrossed in the midline using suture and anchored on the contralateral mastoid. Any extra skin from the skin flap on the lateral neck or midline incision is resected. Patients' postoperative satisfaction and perioperative complication rates were assessed.

[RESULTS] Every patient showed improved jawline and neck lifting. The majority (94%) of the patients were satisfied with the postoperative result. Median follow up was 12 months. Minor complications included hematoma 6%, scar border suffering/dehiscence 7.7%, hypertrophic scaring 1.5% and temporary paresthesia 4.6%.

[CONCLUSION] The proposed technique is a simple, safe and effective method for neck lifting and contouring of the jawline during face rejuvenation surgeries with minimal complications and high patient satisfaction.
