Revisional Extended Reduction Malarplasty for Enhanced Facial Contouring: Reduction of the Periorbital Area.

Abstract

Asians often present with a prominent malar complex, resulting in a relatively flat and broad facial contour. While pronounced zygomatic projection is typically considered esthetically undesirable, conventional malar reduction procedures may not always yield satisfactory outcomes. This is largely due to the predominant focus on repositioning the zygomatic body and arch, with insufficient attention to the periorbital segment. Residual periorbital protrusion can contribute to a persistently flat and square midfacial appearance, prompting requests for further surgical intervention. In this study, we present our experience with the application of extended reduction malarplasty as a revisional procedure to address residual periorbital protrusion following previous reduction malarplasty. The revisional extended reduction malarplasty enabled to improve the flat and wide facial profile and achieve a more 3-dimensional facial profile. It could be a versatile technique in revision surgery, reducing zygomatic protrusion of the periorbital area and enabling further refinement and enhancement of midface contours.
